Documents Required for FSSAI License

The documents you need depend on the type of license:

For Basic FSSAI Registration:

  1. Form-A (Declaration)
  2. Address proof of business premises
  3. Identity proof of ownerBusiness details (nature of food activity)
  4. NOC from local authority (if required)
  5. Source of raw material details

For State FSSAI License:

  1. Form-B (Application)

  5. Recall plan in case of unsafe food

  7. NOC from Central Ground Water Authority (if required)
  8. List of directors/partners with details

For Central FSSAI License:

  1. Form-B (Declaration)
  2. Proof of premises ownership or rental NOC
  3. Identity proof of owner/director
  4. Import-Export Code (if applicable)
  5. Source of raw materials & water testing reports
  6. Food Safety Management Plan
  7. Ministry of Commerce certificate (if 100% Export Unit)
  8. Ministry of Tourism certificate (for hotels)
  9. Any extra documents required by FSSAI
